Thank you for your interest in enrolling in Education Revolution. In order to complete enrollment you will need to:

 1. Complete the enrollment form


2. Email (director @ photos or scans of the following required documents

  • a copy of the student’s birth certificate

  • a completed Health Examination Form completed within the last 12 months OR a letter requesting exemption on religious ground

  • a current immunization record OR a letter of exemption (DH Form 681)


3. Once an email is received containing your student’s documents you will be sent a QuickBooks invoice for your annual fee. 

4. Enrollment is complete, and a confirmation of enrollment will be sent to you. 



We send out invoices confirmations about every 15-30 days.




Alternatively, contact us for the mailing address to send physical documents. 


Your child’s enrollment will be official on the day that all paperwork has been processed. If payment was sent by check, enrollment will be completed after the check clears.


It is important that your email address submitted on the information sheet be checked frequently. You will be sent an email confirmation with your child’s enrollment date for your records; you will also receive emails to remind you of upcoming due dates, as well as any important information. Depending on availability, the cellphone number provided will be used to text reminders of important due dates. Remember, attendance is collected at the end of March, June, Sept, and December.
